Power mobility device provision: understanding Medicare guidelines and advocating for clients

The Centers for Medicare and Medicaid (CMS) issued a new national coverage determination (NCD) for mobility assistive equipment (MAE) including power wheelchairs, handicap scooters in May 2005. CMS then issued a Final Rule in April 2006 that outlined significant changes required for documentation for prescribing MAE. Other insurance providers have since adopted and sometimes modified the NCD criteria and have begun to apply these criteria according to their own interpretations because some of the criteria are vague. In this report, we introduce a case example to show the components of the CMS NCD criteria, what was intended but poorly described in the language, how insurance providers may misinterpret or alter the criteria, and how clinicians can act as advocates.

The System must be as automated as possible so you aren’t spending hours cold calling or following up with people that are probably not interested, then trying to use your sales closing skills to make a sale.
Think of the successful off line businesses such as McDonalds, Tim Hortons or Subway. Do you think they could duplicate those businesses all over the country, setting up new franchise owners to run these businesses, without a SYSTEM that works?
2. BIG Profits- This is important as the advertising needed to result in sales can be quite costly. For some $1000 price point business programs, it isn’t unusual to spend $200, $400 or even $600 to make that $1000 sale. This means you need to advertise to a niche market of people looking for exactly what your offers. Seems so simple a concept, but so may people struggle in building their prospect list.
5. Support and Training- as with any business start up, you need to learn your work at home online business program.Make sure that your business program has ample training and resources so you know exactly what to do to get those prospects to your System. Many of us think of applying the pressure as failing. ‘The pressure is coming. Oh my gosh! What am I going to do? The pressure is coming.’ But pressure doesn’t need to be a negative: I love this quote.

Pressure is a word that is misused in our vocabulary. When you start thinking of pressure, it’s because you started to think of failure. Tommy Lasorda

When you stop and think about what happens in life, pressure is being applied constantly in many different arenas. An example would be rush hour traffic that puts more and more pressure on the highways. As the population grows more and more pressure is added. What happens? Do we, as a society, give up and say ‘oh, well. Too Bad? It’s just going to take you that much longer to transit into work’? Your commute time, instead of being an hour and a half, is going to be 2 hours. And in 3 years from now, it’s going to be 4 hours. Or do we take the action, as a society, as a city, as a person and do something to relieve that pressure? Yes we do something that will alleviate the pressure for all of that traffic coming into the downtown core.

We as a society, as a city, step into success. We do something to alleviate that pressure. We can learn to apply that concept in our life. Instead of taking a look at the pressure as a bad thing we can use it to build something new, a bridge to our future. Do we do what Tommy Lasorda said-misuse that word ‘pressure’ and think of it as ‘oh, I’m going to fail, I’m going to fail’ and then give up? - or do we CHOOSE to do something different and step into our own success?

We all have habits. What is your ‘it’? What is the ‘it’ that stops you? When you start to feel that pressure, what do you do? Next time that happens, I want you to stop and think, ‘what am I automatically doing here’? Then decide to do something different in order to get a different result.

When I was first introduced to this concept, I did not recognize what habit I fell into until I was all the way in it. Stress eating was one of the things I used to do. As I learn to recognize where that pressure point is, what my habit is, what my ‘it’ is, I, can learn how to change it, how to react differently.

I just watched the movie ‘What the Bleep?! - Down the Rabbit Hole’. One of the concepts in the movie is that we, as human beings, are bombarded every second with 400 billion bits of information. We only process 2000, therefore we need to choose carefully which bits we pay attention too. When we succumb to the pressure and fall into a negative habit, like I used to when I was stress eating, we are selecting only a narrow negative portion of information from the 4 billion bits..

I have decided that I am going to choose a different 2,000 bits of information. It really is just a choice.. We need to recognize what our habits, and that we can change our habits. Millions of people have learned how to stop the stress eating habit. Millions of people have learned how to quit smoking. Millions and millions of people have learned how to make their life and Starscapes.com business opportunity better, how to step into success. If they can do it, I can do it. If I can do it, you can do it. It’s all just a simple choice, to make a different decision.

If you are a parent ask yourself, if you choose non-success habits, what are the habits and passions that you are creating in your children? Are you creating a generational curse or a generational legacy? Another great quote is “children have never been good at listening to their elders. But they have never failed to imitate them”. by James Baldwin. Even if you aren’t a parent what about the influence you have on the people around you?

We need to learn to recognize when we are backing off from pressure, when we are moving into the habits that are not success habits (the ones that we need to change). We also need to recognize the habits that we have that are good, that are leading us towards success, leading us to our financial freedom. We need to be aware so that we can make changes in our negative habits and reinforce our positive habits.

We need to learn to recognize that when we wish for something, ask for something, pray for something or when we say affirmations for something, it’s not going to just happen. It’s not going to be handed to us on a silver platter. One of my affirmations is “I am building a team of dream achievers” Now, I know that that is going to take patience and perseverance and that this whole team of dream achievers are not just going to be dumped in my lap. I have to go looking for them and attract them into my life.

As I go down the personal development journey and build my home business I come up against pressure points all the time. When I think back to the pressure points in my life last year at this time versus this year - whoa! - last year’s ones seem really simple now. What happens? We are not given exactly what we want right off the bat. But we are given the opportunity to develop the skills so that we get better. On my journey, I am becoming a better team leader. I am practicing my patience and my perseverance in my own learning so that I become that better learner that attracts, using the Law of Cause and Effect, that team of dream achievers, the team of business builders.

When that pressure is applied in our lives, patience and perseverance, is what we need in order to grow through that pressure point. Once we do grow and find different habits to allow us to get past that pressure point, to allow us to develop the skills, to learn those new things that we need to learn, we come to a better place and we can handle more pressure. We have new skills. We know something different. We react in a different away. We remain patient. We practice the perseverance and can step into success.

OBJECTIVE: To determine how research regarding medicare power wheelchairs and medical scooters and seating selection and configuration could be applied to the pediatric population with spinal cord injury (SCI) and dysfunction. METHODS: Extensive literature reviews were completed on (a) powered mobility options and age-appropriateness for training; (b) manual wheelchair configuration, pushrim biomechanics, and propulsion ergonomics; and (c) cushion selection and use of pressure-mapping devices. The findings in the literature review then were compared with clinical observations from Shriners Hospitals for Children, Philadelphia’s Seating and Wheelchair Clinic. RESULTS/DISCUSSION: A significant amount of research is published on propulsion ergonomics and pushrim biomechanics in adults with SCI. However, this literature review noted that there was limited research available on cushion selection and power mobility configuration. Many of the conclusions drawn from these publications are applied to pediatric patients and used during the decision-making process of wheelchair and cushion selection. However, it is critical to tailor the assessment to the pediatric population to meet their medical and functional needs. CONCLUSION: Although some of the findings can be correlated to the pediatric population, there is a lack of research on wheelchair and seating selection and configuration as it specifically relates to the young person with SCI and dysfunction. Future studies need to be completed.

OBJECTIVE: Quantify manual wheelchair propulsion effort during outdoor community ambulation. DESIGN: Case series. SUBJECTS: Thirteen individuals (12 with SCI, 1 with spina bifida) who were experienced manual wheelchair users and had no current upper extremity injury or pain complaints. METHODS: Measurements were obtained from instrumented wheelchair rims during steady-state propulsion as subjects traversed outdoor concrete sidewalk terrain that included smooth level, aggregate level, and a ramp with a smooth surface. Propulsion effort was assessed using the average propulsion moment, average instantaneous power, and work for both upper extremities. RESULTS: Propulsion effort, captured by the propulsion moment, work and power, varied across ground conditions (p<0.001). Propulsion effort was greater as the rolling resistance increased (i.e., smooth versus aggregate surfaces) and as the inclination angle progressed from level to inclined surfaces. Many people that decide to start a home based business opportunity are destined to fail because their motivation and expectations are completely unrealistic.
Over the years I have helped thousands of people start up their own Starscapes.com business opportunity on the the Internet and I have come to recognize two of the most important factors for identifying people that should NOT attempt to start a home based business.
If a prospective entrepreneur says something like…
“I’m broke and need to make some money…fast!”
or,
“I want to start a home business, but I don’t want to spend anything on it until after I start earning some money.”
…it is certain that they need a “reality check” and should not attempt to start a home business under either of those conditions. I refer to these two comments as the “death sentences” for starting a home based business.
Almost equally important is the need to have realistic economic expectations when undertaking a home based business. There is no magic here folks. A home based business provides many rewards such as working in the comfort of your own home, avoiding a stressful commute, being your own boss, setting your own hours, no dress code, etc., etc., but anyone that is also expecting “instant riches” is sadly mistaken.
There are a tremendous number of home business and work at home opportunities offered on the Internet. Many of these are all hype and try to convince you that it doesn’t take any effort and that you will be making huge amounts of money in very little time (you know, stuff like…”you will be making $5,000 per week, and more, in just a few weeks!”). You should avoid these so called “opportunities” like the plague…if they were for real everyone would be doing it.
Study the opportunities carefully and select the home business opportunity that seems to fit your own particular style or talents. What is a good choice for one person may be a very poor fit for someone else. The common thread that applies to everyone is that you must be “for real” (as indicated above) and the business opportunity itself must be “for real”.

Pressure, Are You Applying Enough to Attain Your Financial Freedom

One of the things that I have discovered in my journey of personal development is that I must make the choice to step into success or I will just stand in failure. I find that it is necessary to apply pressure to myself in order to move forward and take those steps into success. Do you apply pressure to yourself? Let me ask you a question: do you wait until the last minute to finish a task, project or chore? Are you one of those people who accomplish 70% of the project in the last 10% of the time?’

If you said yes to either of those questions then you are applying pressure to yourself. Some of us work better by doing little bits and pieces of the project, task or chore as we go. Others work much better when they are under the gun. When they know they ‘gotta get it done’, ‘it’s due tomorrow’, ‘it’s due in two hours’.. Some people are much better when they are applying that pressure to themselves.

We need to be able to recognize where we fall on that scale and realize how best we react because if we don’t apply any pressure at all to ourselves then we are we are not going to learn, grow and step up into the success that we want in our lives. We all want success in our lives, whether it is emotionally, in relationships, with our family, in our home business, or with our finances. We all want some form of success in our life, if not in ALL of these areas!

Many times, we human beings waste time feeling guilty. By doing this we are wasting our own time and energy. When we get to the point of the deadline for that project, task or chore and we’re feeling so guilty - ‘oh why didn’t I do something sooner? I should have spent more time! Why did I spend my time on that thing and not get this project started?’ As human beings need to learn that it is okay and to recognize that’s how we cope best, we work best under pressure. So stop feeling guilty about it! Stop beating yourself up, because when we stop beating ourselves up and wasting energy on the guilt, we have more time and more energy to invest in the project, chore or task we want to complete.

The other thing that we do, in order to not apply the pressure, is we hide behind our responsibilities. I used to do this so well - hide behind my responsibilities. Many times when I have talked to people about a home based business opportunity, they say ‘oh, well I don’t have the time to do something like that. I am so busy! I have my family. I have my job. I have my charities. I have my hobbies.’ We all have those different responsibilities in our life. But what we need to decide is ‘Is this project/task/business something that I really want to do or rather is this task/business/project going to create a result that I want. You have to begin by deciding what result you want in your life and then arrange your time based on achieving those results. Ask yourself : ‘Are there things I could rearrange or even drop all together to make time to do this?’

Are you applying the pressure to your own life to do what it is you really want to do? Sometimes those tasks or chores are not really things that we want to do, but we want that specific end result so we do them. We want our relationships to work well. We want our financial freedom. We want to be able to take our family to Disneyland. We want that specific end result, the successful life. Today I just assisted my husband in setting up his financial filing system, I sure did not want to spend time doing it, however I want the end result!! The end result of the nice clean, tidy organized paperwork resulting in his taxes being completed on time. I put the pressure on myself, assisted him and now I am very glad I did. Not only did the paperwork issue get better but so did our relationship because my husband felt much supported.

We need to focus on the results we want instead of hiding behind the excuses of ‘I don’t have the time,I’ve got to go to work, I’ve got to take my daughter to soccer. I have got to take my dog to the vet’. Yes, those are all things that need to be done. However when we decide that achieving a certain task/chore/business is a priority that we could rearrange our time or give up some unnecessary activities to free up time if we really wanted to. If we really want that result, we could get it done, it just takes a decision. We will then stop investing in the guilt. we will stop hiding behind the responsibilities and figure out a way to make it happen.

There is a universal law, like the law of gravity called the Law of Growth;. In order to exist, we need to grow. How we grow is our choice. What seeds do we want to plant? Do we want to plant the seeds of not getting something done? Or do we want the seeds of success habits, applying the pressure and stepping into the successful life that we know we deserve. We all deserve a successful life! Ask yourself “am I applying the correct pressure on myself to create my successful life?” I hope you are!

Being committed to a business may mean lack of freedom to do other things and to go as you please.

Disability type influences heart rate response during power wheelchair sport

PURPOSE: The purpose of this study was to determine the influence of disability type on exercise response during power wheelchairs and handicap scooters competition. The secondary purpose was to determine the extent to which heart rate responses during competition meet cardiorespiratory fitness training intensities for the general population. METHODS: Forty-eight athletes who had cerebral palsy (CP, N = 31), spinal cord injury (SCI, N = 10), or muscular dystrophy (MD, N = 7), and were competing in the 2003 Power Soccer National Tournament, volunteered to participate. Heart rate was recorded every 5 s throughout pre-game and game conditions by Polar S610 monitors. Average heart rate (HR) values were determined for GAME and RESPONSE (change score between GAME HR and pre-game HR). The Kruskal-Wallis nonparametric test was used to determine whether a significant difference among group medians existed on the dependent measure, RESPONSE (P < 0.05). RESULTS: A significant difference on RESPONSE (P < 0.05) existed among athletes with CP (29 bpm), SCI (17 bpm), and MD (26 bpm). The median RESPONSE for athletes with CP was 12 bpm higher than athletes with SCI, and this difference was significant (P < 0.01). Further, 22 athletes with CP (71%), 5 athletes with MD (71%), and 1 athlete with SCI (10%) exceeded 55% of estimated HR(max) for at least 30 min during competition. CONCLUSION: Disability type influences the heart rate response to power wheelchair sport, and may affect the ability to sustain training intensities associated with fitness improvement.
